New Client Information Forms

The information requested in Samaritan Counseling Center's New Client Information Forms will be kept confidential and will help each counselor to assist their clients. Each new client must complete the information before the initial counseling session. Clients may arrive twenty minutes prior to their initial session to complete their information forms or they may download the forms from the link below and bring the forms completed to their first counseling session.
